Standard Terms & Policies

Warranty Policy

  • Platinum and Platinum Blue Anti-Reflective Coatings are warranted against daily wear scratches and manufacturing defects (peeling, crazing, or delamination) one time, for one year from the invoice date.
  • Crizal Anti-Reflective Coatings are warranted against daily wear scratches and manufacturing defects (peeling, crazing, or delamination) two times, for two years from the invoice date.
  • Scratch Coatings: Scratch-resistant coatings carry a one-year, single replacement warranty against manufacturing defects only.
  • Exclusions: Warranty does not cover scratches, damage due to mishandling, teeth marks or improper cleaning.

Redoes & Non-Adapt Policy

  • Prescription Changes: Doctor changes are covered for 120 days and eligible for a single redo.
  • Non-adapt remakes involving a change in design (e.g., progressive to bifocal) must be of equal or lesser value; price differences will be credited or billed accordingly.
  • To qualify for a redo or warranty discount, the original invoice number and date of the original job must be provided.
  • When placing the order through our online portal, writing “Redo” in the notes will trigger an automatic no-charge redo.

Billing

  • Terms: Standard payment terms are 20 days EOM (End of Month) from statement. All invoices listed on a monthly statement are due by the 20th of the following month.
  • All special pricing is reserved for customers that pay their statement in full by the 20th. If payments are received after the 20th all special pricing will be removed and you will receive standard list pricing on the following statement.
  • Statements are issued at the beginning of each month and summarize all open invoices and credits.
  • Late Fees: A 1.5% monthly finance charge (18% APR) may be applied to overdue balances.
  • Payment Methods: We accept company checks, ACH, and major credit cards.
    • o Credit Card Fee: Payments made by credit card are subject to a 3% processing fee.
      o ACH or check payments are encouraged to avoid this surcharge.
  • Pair 50 discounts are not valid on package pricing.
  • New Accounts: Accounts are opened upon approval of a completed credit application. Accounts exceeding their credit limit or with past-due balances may be placed on hold until payment is received.

Credits

  • Credits require only the original invoice number so we can verify the job and dates.
  • Credit Processing: Credits are applied to your account balance and reflected on the next statement. Please allow 7–10 business days for processing once the return is received and inspected.

Cancellations

  • Orders may be cancelled at no charge prior to lens surfacing or coating.
  • Orders already in production or completed will be billed at the stage of completion or full cost, depending on the process stage.
  • Cancellation requests must be submitted in writing or confirmed verbally by a Customer Service representative.

Frame Policy

  • Frames must be clearly packaged with the Perfect Optical Rx order form with name and description. If a new frame is damaged by Perfect Optical, we will replace it with the same frame or one of equal value.
  • Perfect Optical Lab is not responsible for any frames lost or damaged in route to our lab. The account will be notified of any damaged frames received.
  • We will take the utmost care but Perfect Optical will not be held responsible for damage to a patients’ own frame, customers shall be notified about any frame damage during processing.

Shipping Policy

  • Carriers: We ship via UPS, FedEx, or local courier, depending on your account location and service preference.
  • Shipping Charges: Standard shipping fees apply unless otherwise specified under a promotional agreement or bulk program.
  • Lost or Damaged Shipments: Claims for lost or damaged shipments must be reported within 7 business days of the delivery date.
  • Rush Orders: Expedited orders are available upon request and may incur additional shipping or handling fees.

Account Suspension & Reactivation

  • Accounts with outstanding balances over 30 days past due may be temporarily suspended until payment is received.
  • Accounts on hold for more than 60 days or inactive for one year will require new credit approval before reactivation.
